Essentials delivered free of charge to help you when you leave hospital
About this service
Our Home from Hospital Pack is a bag of essentials given to you free of charge if you are leaving hospital. This can give you peace of mind, knowing that you will have food ready for you when you return home.
The pack contains three days’ worth of food and groceries, most of the items can be kept at room temperature.
Home from Hospital Packs can only be requested by a health or social care professional. Please see eligibility criteria below.
Service Information
Delivery
Delivery of a Home from Hospital pack can be made to your home, a neighbour, or the hospital reception.
We can only deliver to addresses inside Hertfordshire. Delivery will be made between 11.30am and 2pm.
HILS also provides bags to Age UK hospital discharge teams to store wherever they are located.
Eligibility
To use this service you must meet one of the below criteria:

You have no other means of obtaining essential food items within the first 24-36 hours of being discharged

You are in financial need

You have no other means of accessing support (for example, through family or friends)
Requesting a pack
In order to receive a Home from Hospital pack, we must receive a referral from a health or social care professional.
If you wish to receive one on your discharge from hospital, please speak to a health or social care professional.
